Phosphodiesterase Type III Inhibitors
Cilostazol is a phosphodiesterase type III inhibitor that inhibits degradation of cAMP, which results in decreased thromboxane A2 levels. Thromboxane A2 promotes platelet adhesion. Cilostazol is used to treat claudication by reducing platelet "stickiness." Bleeding and increased mortality in heart failure have been seen.

Class Mechanism of Action
Inhibits phosphodiesterase type III
Inhibits degradation of cAMP, which results in decreased thromboxane A2 synthesis
Decreased thromboxane ultimately lowers platelet adhesion
Increased cAMP also results in vasodilation

Intermittent claudication
Reduced platelet "stickiness"
Vasodilatory properties
Also decreases plasma triglycerides and increases HDL cholesterol, improving lipid profile of blood

Headache
GI Upset
Bleeding
Increased mortality in heart failure
